Going Home


When you are well enough to go home, a patient escort will accompany you from your room to the patient departure area. Although most patients choose to ride in a wheelchair, you are welcome to walk. For your safety, however, you must be accompanied by a patient escort.

Case Management Department
The Case Management Department consists of registered nurses and social workers trained to assist patients and families through the continuum of care including wellness, prevention, acute inpatient and post-admission or discharge. Our staff is knowledgeable of community agencies and services that may assist you after discharge. These services include home health agencies, durable medical equipment, extended care facilities, rehabilitation centers, counseling services, mental health and substance abuse clinics.

We also communicate with your insurance companies, Medicare and Medicaid regarding treatment plans and certification of days. You may contact the Cast Management Department at extension 1597 from 8 a.m. to 4:30 p.m. Monday through Friday. AFter hours or on weekends, call the Medical Center operator who can page the on-call staff.

Pathways Home Care
Pathways Home Care is a licensed home care agency offering intermittent visits and other home care services for clients of all ages. Services are offered 24 hours a day, seven days a week. Home care visits included skilled nursing, in-home aides, IV therapy, speech therapy, respiratory therapy, physical therapy, occupational therapy, medical social workers and the only on-site infusion service in Cabarrus and surrounding counties. Lifeline
Lifeline is a special service provided by Pathways Home Care. Lifeline is a personal emergency response system that allows many people to maintain their independence and live safely in their homes. In the event of an emergency, the Lifeline subscriber pushes a "HELP" button worn around the neck or wrist to instantly alert the Emergency Response Center. The center's staff then arranges for appropriate help. The Lifeline system is monitored 24 hours a day, seven days a week. For more information about Lifeline call 704-784-2110 or 1-800-782-4702.

Hospice & Palliative Care
Hospice & Palliative Care of Cabarrus County provides special care and support at home for patients who are terminally ill. RN's, nursing assistants, social workers, clergy and volunteers are all part of the caring Hospice team. Support is available to all patients and their families regardless of age, illness or ability to pay.

Hospice's Community Grief Counseling Program offers support to individuals or groups of both Hospice and non-Hospice families. For more information, call 704-788-9434.
