Are you or your family members taking multiple medications (including over-the-counter medicines, vitamins, or other herbal remedies?
- Are you having a hard time trying to remember everything that you are taking?
- Are you caring for an aging parent or loved one?
If you answered yes to any of these questions, you are not alone. We understand that it can be challenging to keep track of all your medicines. Below are tools that we have developed for you to help you keep track of your medications.
To download a copy of our medication wallet card, click here. The wallet card is designed for you to record all of your medications, including any over-the-counter medicines or herbals you may be taking. Keep a copy of this card in your wallet, next to your insurance card, remember to always update your information as it changes and bring it (or your medication bottles) with you to your doctor visits.
If you would prefer to record your medications electronically, you can opt to use an electronic tool through iHealth. The iHealthRecord is a free, secure and portable health record that allows you to store your personal health information in one place. The iHealthRecord provides the ability to print out an iHealth registration form, which you can bring with you to your next physician appointment. You can also print out a wallet card to carry with you at all times. This information will be safe and accessible from anywhere and at anytime. You and your providers within the NorthEast Physician Network will be able to electronically communicate with each other through this secure Web site, using the Message Inbox. In just a few minutes you can create an electronic iHealthRecord from any computer.
Creating an iHealthRecord is easy!
