Case Management

The Case Management Department at CMC-NorthEast is comprised of nurses and social workers with an extensive background in many clinical areas. Case Managers are specially trained registered nurses who serve as a patient advocate, providing information and support for patients and their families. They help to coordinate medical care for patients. Our services can be especially helpful for patients and families who may feel overwhelmed.

At CMC-NorthEast, we use a team approach. Case management staff communicate daily with doctors, nurses, therapists, dieticians, and all those involved with the patient to coordinate the plan of care. Patients are then given information in easy-to-understand language. Social Workers provide support resources prior to hospital discharge, arranging for home care, nursing care, or ongoing therapy, if needed.
